Send a written request to ICAI at your regional office stating clearly that she has made mistake in  the articleship form and the same needs to be corrected. Write all the details in the letter i.e. Name, Reg. No., Correct Articleship term and articleship term she has written in the form by mistake. She will get a acknowledgement letter from ICAI that they have corrected the mistake and the problem will be over.
