A, is the shareholder of 500 Shares, due to death it is transmitted in favour of B. Now B has lost the share and requested company to issue Duplicate Share. The Duplicate Share should be in the name of B or A.

Dear Samir,
1. shares were already transmitted to B.
2. so B is the shareholder/member of the company.
3. As B's name appears on the records the company issues certificate in his name.
