What are the forms I need to submit to ICAI for applying for Industrial training other than 104 & 109?

If the organisation is not registered with ICAI than applicaton for registration along with annual report and if it is registered than no other document required...
The org is alreasdyregd. I hv confirmed. What is that i as a trainee need to do.
