is he required take gst no first ??

It will be issued under a Work contract invoice. on which GST will be charged on both material supplied and service provided.

Format for bill is Tax Invoice define in section 31 cgst Act & Rule 46
Note : If your aggregate turnover is not more then 20lac in year , no need to take registration, ...but if you want raise or issue tax invoice , have to have take registration
