Help regarding registration for articleship

Hi guys, 

Can somebody please help me with the procedure for getting registered for articleship. I read it in the ICAI website but still more or less clueless about the procedure.

I have a few querries :-

1)  What is the last date of registration so that I can appear for final exams in may 2014.

2)  What all forms do I need to fill in for articleship and what is the purpose of those forms.
