GST registration cancellation

I received provisional GST certificate on mail even though I did not submit the registration application because my provisional Id and password were sent for old proprietor's pan. I have shut down business and want to cancel registration. What should I do?

i thought the form is GST reg 16...further, where can i find the form? i do not see it on the GST website.
