They also have 1 employee in Karnataka providing services and has taken up a small space for rent to provide service. All the billing is from Gujrat. Should they take Gst registration in Karnataka as well?

I think it is better to take registration in the light of Colombia Hospital AAR as cross charging may apply

Yes... You will go to register in Karnataka.
It is better to take Registration in the State of Karnataka as well since you are already registered in the state of Gujarat and under GST Regime, Registration is State wise.
