GST Migration for centralised Service tax registration

if assessee is having offices at different location/states in India, it is necessary to obtain seperate GST no for each state and if answer is yes, how to get provisional I'd and password

log on to www.gst.gov.in and apply for GSTN and yes you need to apply different GSTN for each state
