Buddy i have completed articleship with pure rules and regulation thus i can give u my office format which i am sure is provided by ICAI.

Its like as follows:

Top middle: Firm Name with details

Top Right corner: week ended

Seven coloumns : from left side respectively indicated for 1- Date, 2- Client, 3- Works to be done, 4- Date, 5- Client, 6- Works done, 7- Initial of officer.

Down Left corner - Signature of Article/Audit Asst.

Middle Down position - Signature of Incharge

Down Right corner - Signature of Principle
