Form no. 112

i'm doing CS along with CA registered in 2006 now i have applied for the extension of registration period. which date will i write new or 2006?

i'm not getting what's the duration of CS course? what to write in subjects taken for the CS course?

where i have to submit one copy of 102, 103 & 112 by post or at the icai chapter?

and i have paid 9k at the time of ipcc registration, will i have to make any DD or it's not required?
