Form 10ia for disabled person

I am person with disability. I am locomotor disabled with 40% disability. I have medical certificate from civil surgeon.
But when I claim deduction for this, form 10ia need to be filled and in that form no option is there for locomotor disability.

there are only 3 option namely cerebral palsy , austim and multiple disability.

what should I choose.
