Form 104 & 109 late submission

Hello Everyone,

I joined industrial training from 1st April 2014 with the consent of my previous principal.As per ICAI rules,I should have submitted form 104 & 109 within 30 days from joining industrial training and leaving my old firm.However, I failed to do so,as I met with a minor accident and later my principal was out of town.

Please members guide me as what should be my action and necessary formalities to complete the documentation with institute.
