Error in uploading

I am uploading incorporation documents of a company in MCA portal. But it shows error that DSC is not attached to any documents. Actually i have attached all the necessary dsc. kindly advice me on the error

I have again downlaoded spice filled form and uploaded again. still error continues

Kindly remove all history from the browser and than try again
