Disclosure regarding interest in other concern

I am incorporating one person comoany. While filing form 32 eSpice, details of interest of proposed director in other entity   has been asked. In my case director is interested in a patsanstha i.e.type of trust or pedi. My query is

1. Whehther he has to disclose interest as he is director in that trust holding .022% shares

2. If yes which form need to be submit along with 32 and what be the additional process.
