can anyone please share the format of declaration to be given by ID in compliance with rule 6(3) of ompanies (Appointment and Qualification of Directors) Rules, 2014.

I would request you this format can be downloaded directly from the Google and there are various format/templates are available there. And even then you require more clarity you can access the icsi publication where you can get easily all relevant information for authentication.

You can check MCA public data of listed company, Where DIR-12 is filed for appointment of I/D ,however the declaration is simple what provided under Regulation 16 of LODR or Section 149 of Companies act, 2013 and relevant rules made there under, If you go through them then it simple and you can also draft it. Also not to include Details about inclusion of name in Data Bank of IICA.
