And even then you require more clarity you can access the icsi publication where you can get easily all relevant information for authentication.
You can check MCA public data of listed company, Where DIR-12 is filed for appointment of I/D ,however the declaration is simple what provided under Regulation 16 of LODR or Section 149 of Companies act, 2013 and relevant rules made there under, If you go through them then it simple and you can also draft it. Also not to include Details about inclusion of name in Data Bank of IICA.
