Cpt marksheet

I started my articleship this year Now i come to know that CPT Marksheet is to be submitted along with the form 103. And my CPT marksheet is lost.

Have I to submit marksheet ?? Its compulsory ??

If its compulsory then i have to make application to icai to provide me duplicate marksheet, and want to know tht on ICAI site it is given that   https://www.icai.org/new_post.H T M L?post_id=653 at this link which form i have to send to the ICAI and please also mention the fee which i have to pay.

and please tell me what is the diffrence between the Marksheet and Pass Certificate in context of CPT ?
