Condonation fee DD in favour of

Hello Friends,
This is Vipul Jain. The proposed date of completion of my articleship was 14/09/2017 but due to my examinations i couldn't get my Form 108 signed by my principal. It is unsigned till date. I wish to know in whose favour and at which place the DD is payable in respect of condonation fee?

Also, will the condonation letter be written by the principal himself or by me?
Pls reply as soon as possible. I need to submit it at the earliest.
