Claiming 80U

In order to claim 80U benefit of Rs 75,000 for disabled employees, one must submit the medical certificate indicating the disability together with the income tax returns as per Section 139 for the relevant AY. Can anyone describe to whom and when to submit?

You may claim 80U based on the certificate.

Do I later need to get involved with the assigning officer later as well ?

80U is based on the certificate and not based on the actual payment
