Can both "preventive health checkup of 5000/- under 80d" and "section 80dd" together can be claimed

Hi,

Can both "preventive health checkup of 5000/- under 80D" and "section 80DD" together can be claimed?

pls note nothing else claimed under 80D as there is no insurance taken.

When submitted for above both, not considered "preventive health checkup of 5000/- under 80d" and got "notice of demand u/s 156 of the income tax act 1961" to pay 

Pls let me know :

- is this  demand needs ot be paid and resubmit ITR form ?

- or can go for rectfication ? if so what documents to submit?
