CA certificate for remittance using Form A1

Where imports are made in non-physical form, i.e., software or data through internet / datacom channels, a certificate from a Chartered Accountant that the software / data has been received by the importer, may be obtained. This certificate has to be attached along with Form A1 and sent to the AD.

Can someone share the format in which the Chartered Accountant has to issue this certificate ?
