Ca articleship transfer

hii, If you havn't completed an year of articleship then the formalities are to just fill up the form 109. 
take signature of your principal. 

and meanwhile search a new CA firm in mumbai, go there are fill up forms 102 and 103(in triplicate) again as you did earlier .

So you have exactly 30 days time from the transfer/termination date you can submit your 109 and 103 form to ICAI. 
That's the procedure.
