Simply fill form 109 with that no documents need to be attached. But when you join new firm then resubmit 102&103. But just wait for 10-15 days and use this time for searching suitable new firm so that you can start A'ship back to back after trf

Go through our CA website for termination and transfer whether any restrictions are placed and the procedure to be followed. Procedure like  filling Form 109 with the reasons for termination, how many hours works there, what subects you have dealt there. Franklin on Form 102 on the next day of termination. One thing you must be sure is "You should not be out of Service". Consult your friends those taken termintion and rejoined. I wish you all the best 

take 109 from old employer,send it to icai with in 30 days from the date of termination along with a covering letter from ur employer letter head

join in with ur next employer and send 103 to icai with in 30 days
