You need to file Form 108 with institute

IS THERE ANY PERMISION WILL BE TAKEN FROM ISTITUTE REGARDING TERMINATION

No, you just need to complete the form and send it to the institute.

What is the minimum time in which we must have to submit the form after completion??and what is there is extension?
