Form 108 is to be filled and sent, it can be downloaded from here:

to where i should send it??? and just 108 will do it... so simple???

You have to send it to your regional council. Yes, it's simple as that! Throughly go through the form and then fill it, you can find some filled sample form/s here in some thread/ topic.

actually sid there is form no 119  which need to be filled and submitted to ur CA and a copy of that is to be send to Secretary of ICAI

Form 119 says, Form for request by the Articled Assistant to his Principal for issuance of Service Certificate in event of Completion of articles.

You want to fill up 119 to request your CA to sign your form 108? Why don't you get your form 108 signed directly? Form 119 is to be filled when the CA insists on having more paper-work or, is troubling to sign 108..

See whatever you prefer or better off ask your Boss what he wants. BTW, it was you who asked the question, didn't you..

Am abt to finish my Industrial Training soon..so is same form(108) applicable or diiferent form..????

Form 105 is the Service certificate for Industrial Training.
