Application for export house

I want to apply for Export House, for the same I referred to www.dgft.gov.in website, however I could not gather sufficient information for procedure to make application of the same. I would like to get relevant information i.e. webpage from where I can get below mentioned my queries:

1.     Whether double weightage is eligible to only agro based products or any other organization is eligible for the same?

2.     Is there any prescribed form for application of export house? At which address the same is available?

3.     What are the documents to be submitted with relevant form for application of export house?
