Articleship extention under other principle

I as an article assistant served my principle for 3 years from 10th feb 2015 to 09th feb 2018, but due to excess leave I have to extend my articleship by such excess leave. I want to serve the extended period under some other principle, can i do so? If yes which form should i sign up from my previous principle and from my new principle.
