Compliance of form 107

I have recd. a letter from the institute in reply to submission of form 107 stating that it needs the original deed duly executed on a non-judicial stamp paper for the period of extension. I want the matter that is to be printed on the stamp paper and i am unable to find any specimen of such deed. Kindly help, its urgent,
