We have lost F form original copy Mar,2009 but haveing duplicate & coutnerfoil copy with us.
Kindly you are requested to advise for getting F form original or attested copy form sales dept.

first you shoud make a FIR in near Police Station

second you should make a affidavit stating that loss of F form with notary.

the same copy handover to the sales tax department.

they will issue the Duplicate F form
