I have submitted Form-108 but did not get the certificate after enquiring ICAI they told me that as my Principal has changed her Name so her signature does not matches. So I have to submit a letter written by my Principal under her Letter Head giving an undertaking for changing her name.
So please can you guide me on the format of the letter??

I,AACC, aged 31 years, daughter of Mr. BBB , member of The Institute of chartered Accountants of India with membership number 012318 solemnly affirm and states as follows:-
That I am conversant with the facts of this issue and competent to make this declaration.
That Miss. SONIA , aged 21, daughter of Mr. ......,residing at........ was a student of Chartered Accoutancy with SRN.456789.
That I was the principal of the said Miss. Sonia to undergo her Articleship training of Chartered Accountancy Course.
That I have entered my name as AA in the deed of articleship and in the article ship registration form no.102 executed with the Institute of Chartered Accountants of India.
That I have changed my name as AACC, during the tenure of articleship of the said Miss. Sonia, consequent to my marriage with Mr.CC.
That AA is my maiden name and AACC is my name after marriage.
That AA and AACC is my name and represents one and the same person.
That the above is my true and correct declaration and I have concealed nothing and no part of the above declaration is false.

PS:- SONIA Ji . The reason for name change of your PRINCIPAL is given as MARRIAGE. You may edit the above declaration by stating the correct reason for the change and submit to ICAI. All the very best.

One more doubt I wanna ask.Sir My Principal has also changed her Firm few months before my articleship. But ICAI did not mention anything regarding such change. So do I need to mention the same in the above mentioned letter as ICAI did not ask anything about it?

Sir My Principal is married so many years ago just recently she has changed her surname so is there any need to submit document regarding marriage or the undertaking letter would be enough ???
