Mr Ashok Visit the web site Gujarat Government Commercial Tax department(www.gujcomtax.gov.in).In that click on top items list "forms",then 'CST',then you get the list of CST forms including 5B,click on it, Form 5B of Gujarat CST rules will open.It may be in gujarati or English,take a print and use it......mjk
