Enclosure with ca final exam form

16 August 2016 I am a CA Final student and going to appear my final exam in nov 2016.
My articleship has already completed 1 year ago.
Is it necessary to send Attested copy of "Certificate of Service" to ICAI with exam form
as I have already sent it last time i.e.with exam form of May 2016....
Please guide me whether it is necessary to send Attested copy of "Certificate of Service" to ICAI every time with exam form?
Reply me as soon as possible as it is urgent
