25 August 2016 I am in my third year of articleship. On the account of severe headaches (migraine) and chest pains doctors have advised me for rest so i have decided to take termination on medical grounds. I have discussed thiswith my principal and he agrees to terminate my articleship. My doctor has agreed to give me medial certificate (Government Hospital). I have gone through the procedures of termination and I have certain queries regarding that:
1) Whether request letter (to be send by article trainee to the institute) for termination be hand written?
2) Whether consent of principal be on his letter head or on a plain paper or his signature on my request letter will amount to his consent?
3) What may be the grounds on which institute may reject my request?

29 August 2016 In continuation of my above query I have few more questions to ask:
1) Whether the original medical certificate (to be treated as a proof of termination on medical ground) is required to be self attested by the article trainee?

2) Whether in case of termination on medical grounds, termination can only be taken from any date after the date of receiving approval letter of the institute because doctor has suggested me for immediate rest and I may not be able to carry on the office. Moreover the medical certificate would also include the date which will be of these days only i.e. before the date of approval letter of institute.
Further I will also mention the date from which I require termination as per doctor's recommendation in my request letter.

30 August 2016 Dear Richa, First submit your termination form and do thing entering else. Yes u have to submit medical proof but article trainee signature is not required on it. Further, days will be decided by icai Hope your query is resolved Kindly like if my response has helped you

30 August 2016 Sir how can I first submit registration form because firstly I have to seek permission from institute. Accordingly I want to ask that whether the termination date can only be any date after the date of approval letter of institute?

03 September 2016 It means that on a plain paper if I get the consent of my principal duly signed by him then there will be no issues.

Yes u can take it on plain paper also but it would be extra advantage if it's on letter head.

So don't worry if u have taken on plain paper then also it will be accepted

15 September 2016 Sir I had sent my request letter with all the other required documents to the institute for termination on medical grounds but i haven't received the reponse from institute. almost 15 days have passed. In how many days it is expected to get the response from the institute as i am worried that whether institute will accept my request?
