Return of allotment-form 2

Dear frnds

Can any one send me filled up latest Form -2 (Return of Allotment) for my reference.

or suggest me on

While filing  column 3 in Fom-2,  whether total amount paid on application be equal to total nominal amount of shares alloted (No premium/discount on issue) ????

Eg. 1000 shares alloted F.V. Rs. 10, Total nominal amount Rs.10,000/- whether this total amount can be paid on the date of application of shares ??????
