Whether i can apply for more than one business in a single form??

I want to start trading of home based medicines in Mumbai and also run an OLA cab in my name. I want to know if I can apply for a Gumastha licence for these two business projects in a single application??...If yes, what is theprocedure to go about the same??

Gumastha licence is shop estiblishment licence
