Whether VAT will be Levied on (Sales+ Entry Tax) or Only on Sales Value ?With Special reference to Orissa State VAT.

VAT will be calculated on Sale Value only, after that the Entry Tax will be calculated i.e. (Sale Value + Vat Value)
Plz provided me u.p. entry tax return form in excel format.
