Regarding form 108

Hi,

I am about to complete my artcileship and  i have query regarding form no.108  as i am a PCC Student and my period of ArticleShip  is 3.6 years and form 108 contain Columns for  3 years only . ( i.e. to fill the Details of Work done).so please help me  to which form should be filled or there is any other form.
