Itr form

Dear Sir

One of my clent had a Profeesional income, salary income, House property income  and as wellas capital gain income,

in which ITR Form we can file  his income tax return?  The profit is declared by opting presumptive income u/s 44ADA.

Is IT form 4 or Form 3?

Please suggest.
