Intimation U/S 143(1)

I received the Demand OR intimation U/S 143(1) for A Y 2009-10, of INR 78K, I field the Return Online, I analysed the Demand and found that i made a mistake in filling the xls sheet. I made mistake in following things..

1.    Wrong consideration of “Income From Salary”,  I did not deduct the Professional Tax.. in this case

2.    Non Consideration of “Gross Total Income ( After Intra Head Adjustment)”,  By mistake i had INR 0 in the xls sheet

3.    Non Consideration of “Loss of Current Year Adjusted” , by mistake the value appeared in the Loss to be carried forward.

4.    Non Consideration of “Gross Total Income including income from other sources”, By mistake i had INR 0 in the xls sheet

5.    Non Consideration of “Total Income After Deduction”, By mistake i had INR 0 in the xls sheet

6.    Consideration of “Loss in current year to be carried forward”, this amount has to be considered in Loss of Current year

I am trying from last 2 days to upload the rectification online but the tool contantly saying me that data does not match.. i enquired on CPC call center they are telling me that entire india is facing the same problem from the tool/server. send us the hard copy OR application.

Can you please help me out which document i should attach and format of the application for rectification U/S 154.
