Please let me know which ITR i have to file.

Number and types of returns is reduced for FY 2016-17 wherein,
In your case, ITR-4 needs to be filed.

If I have income from salary in India and income from USA as Freelancer, FD Interest, Interest from Tax Free Bonds and Shares dividends then which ITR form I have to file?
