form needed to import goods in mp state

i import agricultural implement from china through mumbai port and now all the custom clearance is completed and now we want to transfer this to madhya pradesh from mumbai .but transporter need form 50 or form 49 from us .as implement is for personal use please anybody let me know waht to do and how to get these form VERY VERY URGENT PLS

sir do we need to pay any vat tax or any other tax on agriculture implement at check post of mp ,please explain in detail all precedure thanks for ur help

i found after check, if the imported item is above mentioned the rate of duty is 4% ok
