26 February 2017guys , i have downloaded the pdf after filling my exam form and my photo and signature and address are appearing on it , but still on the 2nd page it mentions a blank space to write the candidates name and fathers name on the first line and a blank space for members sign at bottom right , plz clarify asap

If your photo and signature is already there on the form then no need to post.
