Dear Parayil,
I think you are normalising a Dormant Company so that filings can be done. As far as my knowledge is concerned we dont need any affidavit for that. Only a declaration or letter is needed to be given signed by the Director of the Company as an attachment.
To confirm wait for the comments of experts.

You need to file eForm 61 with following attachments :-
1.) An application showing the cause of delay in filing the annual returns.
2.) A Notorized Affidavit in the support of the cause of delay.
On approval of the Form 61, you have to file All annual filing forms winthin 21 days from the date of approval of Form 61.
If you are unable to file annual filing forms within 21 days then the status of the company will still be inactive.
