Dear Jagadish,
You will have to file Service Tax Return in Form ST - 3.
In the Service Tax Return, you will have to mention the following things:-
1) Name of the Assessee
2) Address of the Assessee
3) Constitution of Business
4) Name of Taxable Service provided by the Assessee
5) Whether Abatement on the Service claimed by the Assessee
6) Gross Amount received for Services provided
7) Rate wise breakup of Service Tax
8) Bifurcation of Service Tax, Education Cess and Higher Education Cess collected
9) Adjustment of Service Tax paid in Advance, if any.
10) Bifurcation of Service Tax, Education Cess and Higher Education Cess paid ie. Cenvat Credit availed.
11) Details of Service Tax paid (Amount in Rs., Challan Date, Challan Serial No.)
Now you will have to file the Service Tax Return online if you have paid Service Tax more than Rs. 10 Lacs in the previous Financial Year, if not then you can file it manually.

If you are filing the Service Tax Return manually, also attache the Photocopies of the Service Tax Payment Challans along with the Return.
