Audit Query/ Notebook

Is there any register or proper format where queries can be written/recorded in legible manner for easy working

I am searching for any format or dairy in which we record our audit query while auditing

as till now all the query are recorded in either notebook or loose sheets

I am asking dairy/register like thing which have proper format to record query for better maintenance of Working Papers
