Articleship completion query urgent

I have completed my articleship period on 22nd June 2012. I just wanted to know that what forms i have to submit to ICAI office. Only Form No. 108 or others also????? what is Form No. 119? Do i have to submit it to ICAI?

I have served 2 years of articleship first and then i was terminated by mutual consent, then again after 3 months i Joined under same principal, so all these details do i need to fill in recent Form No. 18?

Just submit form 108 with details of latest firm.

You have to submit only form 108 with details of articleship training in second firm.
