29 April 201715G & H has to filed quarterly . If suppose i have received the declaration on 01/04/2016 and i am paying quarterly interest to the lender, in the amount paid column( in the form , which amount i have to fill. whether the amount for the whole year or for only first quarter?

The form also requires to fill " Date on which income has been paid or credited " . Shall i fill as 31/03/2017 ( Because i am paying / crediting interest in 4 qtrs )
