Uploading unpaid/unclaimed dividend

The recently introduced amendment requires the uploading of unpaid and unclaimed dividend and the form requires the names, middle name and last name of the concerned shareholder. Many companies have not obtained data in the above format. It also requires similar details for father's name, which will not also be available. Under the circumstances, how to fill in the form. Will it be enough if the name is filled in the first column only, indicating in the other columns 'details not available'.
