Share transfer in a private company

A private Limited Company has lost share transfer forms submitted to it. These transfer forms are very old. What options does the Company have?

 

Also what if a transfer of shares has taken place in the past but not recorded in the books. What is the process the Comany will have to do? will it have to file any condonation in the court? please advice.
