Dear Sir,

U need to attach consent letter of director alongwith the form 32. There is no specific format of consent letter you may write it on behalf of the director in simple language and take sign from them and submit it to ROC.

Some of the important information which you need to fill in that are:

Name of Director,

Personal details of deirectors like adress, father name and any other

Undertaking or consenting to act as a director of the company.

I think u need to use new form 1 and submit it with all the attachements you've attached with original form 1, and in form 32 you just have to select the executive or non-executive and resubmit them.
